checkout short code doesn't display past Price Breakdown header

Our Checkout page just has the checkout shortcode on it.
The page is setup in Accommodation Settings as the checkout page.
Reservation buttons are not disabled.
After clicking Reserve from the search results screen it goes to our checkout screen and lists:
Booking Details
Accommodation #1
Guests
Full Guest Name
Coupon Code
Apply Button
It just has the header: Price Breakdown but no more information. It doesn’t show price breakdown and doesn’t have place for guest to fill in their contact and payment information.
We have Braintree selected as payment gateway and Confirm on Payment selected. Our sandbox payment information is entered right now and Test Payment is also enabled.

How do we get the rest of the checkout page to display? Will it not display with sandbox accounts?
